Q:  How can I change my Primary Care Physician/Women’s Health Care Provider?

A:  Call Harmony Member Services at 1-800-608-8158 (TTY 1-877-650-0952), Monday–Friday,8am-      5pm.

 

Q:  What should I do if I can’t reach my Primary Care Physician?

A:  Call Harmony Member Services at 1-800-608-8158 (TTY 1-877-650-0952) and we will help you reach your doctor.

 

Q:  How can I get a prescription for my children or me?

A:  Use your Healthcare and Family Services (HFS) Medical Card to get your prescriptions.  You can get your prescriptions from any pharmacy that takes that card.

 

Q:  What if I lose Harmony ID card?

A:  Call Harmony Member Services at 1-800-608-8158 (TTY 1-877-650-0952) and we will send a new ID card to you.

 

Q:  Who should I call if I move or my phone number changes?

A:  Call Harmony Member Services at 1-800-608-8158 (TTY 1-877-650-0952) so we can change your address and phone number. Then you will get all the information we send to you.

 

Q:  How can I schedule a ride to my doctor’s appointment?

A:  Call for transportation at 1-888-684-2026 (TTY 1-877-650-0952) at least 48 hours in advance of your appointment. A ride will be scheduled for you.

 

Q:  How can I get my Harmony +10 Personal Care Items?

A:  Just follow these steps:

  • Choose what you and your family need.
  • Call Member Services with your order.
  • The toll-free phone number is

That’s it! Your order will be mailed to your home at NO CHARGE to you!
